Potential SPIRIVA HandiHaler Design flaw

I starded using the SPIRIVA HandiHaler a few weeks ago and recently noticed a small particle of the capsule fall out of the unit when emptying it. The particle was created by the puncturing device within the unit and could have easily been inhaled into my lungs. How many times on other occasions of use could this occur and what danger does it present, especially over a long period of use, I do not know. Has anyone else encountered the same problem and is any information vailable from the manufacturer related to this potential danger?
