Chest pain

Any Ideas?

The following are symptoms
1. chest pain while walking. Starts after 1/8 mile flat land no hills
2. sharp pain is all across upper chest
3. at the same time I get painful numbness in both arms from finger tips to shoulder. Pain feels like when
you hit your funny bone only stronger.
4. Pain would go away in a couple minutes after taking a .4 mg nitro pill but recently I’ve had to take two and it might take 5-10 mins to stop.

A little background info

I’ve taken three stress tests in the last 4 yrs. The1st one I had just completed and as the machine was stopping I passed out.
Test two I got ½ way through and passed out without any warning nothing showing on instruments. I was ash white.
Test 3 only lasted 2 mins and I was out cold with no warning. This time I went to hospital and had a heart catherization. Which did show a blocked artery but it has already bypassed itself and blood flow is good.
I’ve had numerous tests

Stress echocardiogram carotid ultrasound
Tilt table test heart catherization
Catscan of head chest xray
Radioactive dye in air and blood with xrays
Ekg’s numerous blood tests
Mri of head neurology scan
Endoscopy colonoscopy
Eecp mri of stomach area
A breathing test where you exhale but I didn’t finish as I passed out on it also
I wore a cardiac monitor for a month then a different cardiac monitor for 48 hrs
Everthing has been negative
Blood pressure at time of chest pain
164/84/107
158/77/107
150/68/103
151/76/116

My history 61 yr old white male 6’ 1” 223# Diabetic type 2 a1c runs from 5.9 - 7.0

Medicine
1000mg metformin 2 x a day 100 mg/ 50mg 2x metoprolol
100mg januvia 1 x nitroglycerin patch .4 mg daily
145mg tricor 1 x 75 mg plavix 1x
40mg lipitor 1x 1000mg ranexa 2x
34ml lantus insulin 1x 40mg nexium 1x
